As a Registered Nurse Case Manager licensed in Illinois, you'll play a key role in guiding patients through the complexities of the healthcare system. Working remotely, you'll use your expertise to enhance member satisfaction, optimize cost-effectiveness, and provide compassionate support as part of a collaborative healthcare team.
What You'll Be Doing
Analyze data to identify members eligible for population health management programs.
Deliver care that is timely, safe, equitable, and focused on each member's unique needs.
Manage case assignments, including outreach, monitoring, documentation, and closure.
Collaborate with team members and clients to achieve exceptional outcomes.
Support members in overcoming barriers to health autonomy by addressing social determinants, motivations, and psychosocial factors.
Educate members on navigating healthcare systems while promoting quality and cost-effective care.
Maintain confidentiality of all medical records, data, and computer systems.
Participate in quality assurance and utilization management meetings, preparing materials and following up on tasks as needed.
Provide on-call support during off-hours or weekends when required.
Stay updated with nursing best practices and meet continuing education requirements as defined by the Illinois Practice Act.
Perform other duties aligned with program requirements and organizational goals.
What You Bring
Active Illinois Registered Nurse license.
At least five years of diverse experience in healthcare settings.
Expertise in case management and chronic condition management.
Familiarity with utilization review, quality improvement, managed care, and community health.
Experience with remote or telephonic healthcare delivery.
Strong clinical judgment and excellent communication skills.
Organizational and problem-solving abilities to thrive in a fast-paced environment.
Proficiency with MS Office (Word, Excel, Outlook) and multiple EMR platforms.
A private, HIPAA-compliant home office with a secure high-speed internet connection.
Preferred Qualifications
Certification in Case Management is a plus.
Compensation and Benefits
Salary: $70,000 - $75,000 annually (dependent on experience, licensure, and qualifications).
