What You'll Do
As a Care Coordinator, you'll be the key liaison between patients, healthcare providers, and insurance partners, ensuring seamless, compassionate care. Your responsibilities will include:
Patient Care Management:
Assess patients' medical needs to understand their full clinical picture.
Coordinate care plans with hospitals, pediatric facilities, home health services, pharmacies, and other providers post-discharge.
Facilitate discussions with patients and their families to ensure they understand treatment plans and next steps.
Health Insurance Navigation:
Review patients' health insurance benefit plans to confirm coverage options align with their needs.
Advocate for patients to secure access to necessary services and resources.
Care Coordination & Communication:
Build strong, collaborative relationships with healthcare providers and specialists.
Communicate regularly with healthcare teams to ensure coordinated care and smooth transitions.
Address clinically complex situations and respond to patient needs within 1-2 business days.
Documentation & Team Collaboration:
Maintain detailed records of patient interactions and care plans.
Participate in team meetings to discuss patient needs and any barriers to receiving care.
Operate in problem-solving mode, addressing challenges with creativity and compassion.
What We're Looking For
We're seeking a compassionate, detail-oriented professional with:
Education: Bachelor's degree in Nursing (BSN) or Social Work (BSW) with a clean and active MA license.
Experience: At least 3+ years of experience in case management, managed care, or care coordination.
Tech-Savviness: Ability to work remotely with confidence in virtual communication and documentation.
Problem-Solving Skills: A proactive, solutions-oriented mindset to overcome obstacles in patient care.
