Key Responsibilities:

Develop, implement, and monitor personalized care plans that align with patients' health and wellness goals.

Serve as the primary point of contact for patients and their families, offering regular updates and addressing any concerns.

Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive, coordinated care.

Utilize health coaching techniques to assist patients in managing chronic conditions and making lifestyle changes to improve health outcomes.

Maintain accurate and timely records of patient interactions, care plans, and any changes in health status within the electronic health record system (EHR).

Qualifications:

Graduation from an accredited school of nursing.

Active Montana Nursing License (RN or LPN) or a current temporary license.

Previous experience caring for patients with chronic diseases is preferred.

Strong organizational and communication skills with the ability to collaborate effectively with a multidisciplinary team.

Certifications & Requirements:

Valid Montana Driver's License.

CPR Certification.
