LeeKO Home Health LLC is a trusted provider of home health services, committed to delivering exceptional care and improving the quality of life for our patients. We are seeking dedicated and compassionate Physical Therapists and Physical Therapist Assistant to join our growing team.

Responsibilities Physical Therapist ( PT):

• Provide in-home physical therapy services to patients in accordance with physician orders and care plans.
  
• Assess patient needs and develop individualized treatment plans.
  
• Educate patients and caregivers on therapeutic exercises and mobility techniques.
  
• Document all visits and progress notes accurately in Synergy EMR system.
  
• Collaborate with interdisciplinary team members to ensure comprehensive patient care.
  

Physical Therapist Assistant (PTA)

Responsibilities:

• Assist PT in implementing treatment plans.
  
• Monitor patient progress and report to PT.
  
• Educate patients on exercises.
  

Requirements

Qualifications PT:

• Current Florida Physical Therapist license.
  
• Minimum of 1 year of clinical experience (home health experience preferred).
  
• Strong communication and organizational skills.
  
• Ability to work independently and manage time effectively.
  
• Reliable transportation and valid driver’s license.
  

Qualifications PTA:

• Florida PTA license.
  
• Minimum of 0-1 year of clinical
  
• Home health experience preferred.
