Position Purpose:  As a Medical Social Worker, you will work under the direction of the designated physician and clinical care providers, work collaboratively with multidisciplinary team members, patients, families, and community agencies to identify appropriate community resources to achieve desired patient outcomes.

Hours/Shift:  Full-Time / Days M-F 8:00a – 5:00p

Key Responsibilities:

  • Status and Level of Care Management
    • Collaborates with the interdisciplinary team members to ensure comprehensive care for at-risk patients identified by the case manager.
    • Provides education/consultation to the nursing and educates patients regarding regulatory and insurance processes and requirements (i.e. AHCCCS and Medicare), ensuring requirements are met.
    • Provides social services by utilizing knowledge and skills necessary to effectively meet the growth, development and continued clinical needs for patients of all ages from newborn through geriatric.
  • Psychosocial Interventions
    • Screen patient's and family's psychosocial risk factors through evaluation of prior functioning levels, reaction to illness, ability to cope, and appropriateness and adequacy of support
      systems. Identifies any psychosocial issues/needs and identifies and assists patients requiring assistance with eligibility determination for social programs and funding sources.
    • Develops a collaborative care plan to facilitate provision of services/referrals that are needed to meet the needs of the patient during hospitalization.
  • Discharge Planning
    • Participates in and facilitates discharge planning activities for complex patients in order to ensure a timely discharge, facilitates placement/transfer of patients, arranging necessary post-discharge care services and facilitating the development of a discharge plan of care for high-risk patient populations. 
  • Patient and Family Support
    • Provides intervention in child abuse/neglect, domestic violence, guardianship (temporary/permanent), adoption, mental health placement, advance directives, adult/elderly abuse, child protection and sexual
      assault. Reports any suspected abuse to appropriate agency.
    • Works with public and private fiduciary for decision making when patient unable.
    • Communicates with physicians, nursing staff, and case manager all new/changed information related to patient/family decisions and plan of care.
  • Compliance and Development
    • Ensures safe care for patients, adhering to policies, procedures, and standards within budgetary specifications, including time management, supply management, productivity, and accuracy of practice.
    • Performs social work services and protects patient rights maintaining confidentiality and within legal, ethical and professional standards as well as departmental policies and procedures. 
    • Participates in designated Quality and Process Improvement initiatives and in interdisciplinary rounds to ensure full collaboration of quality care for the patients identified as having complex needs.

Qualifications:

  • Education: Master’s degree in Social Work (MSW)
  • Certification/Licensure:
    • Level one fingerprint clearance card, required within 20 days of hire
    • Valid AZ Driver’s License with clean driving record
  • Experience/Skills:
    • MSW with at least one year of healthcare social work experience
    • Knowledge of post-discharge resources and patient needs
    • Strong communication and teamwork skills; effective problem solver
    • Adaptable to diverse backgrounds
    • Proficient with business office technology

Work Requirements:  Ability to handle multiple priorities simultaneously, sit, stand, bend, and walk for entire scheduled shift. Expected duties have routine potential for exposure to blood, body fluids or tissues and possible exposure to volatile/violent patients or family members.
