Legacy Oaks of Sacramento Assisted Living and Memory Care, located in Sacramento, CA is seeking a passionate and experienced Health and Wellness Director to manage our Health Services program and ensure high-quality care is being provided to the residents in our community.

Position Summary:
The Health & Wellness Director is responsible for providing oversight of the resident health care service needs. The HWD is also responsible for the overall medication delivery system and the in- service training of the Health Services department.
REPORTS TO EXECUTIVE DIRECTOR
Position Responsibilities:
- Adheres to and conveys the philosophy and mission of the Management Company.
- Is aware and knowledgeable of the rules and regulations for Residential Care Facility and Memory Care in the state in which employed as well as the regulations of the state’s nursing commission.
- Participates in the move-in and ongoing assessment process per state regulations and Management Company policy as appropriate and/or requested.
- Stay within budgetary guidelines.
- Maintain oversight of the schedules for both Assisted Living and Memory Care. Final sign off prior to posting live. Ensure staffing ratios are appropriate to facility need and adjust as needed.
- Coordinates health care services using the HWD audit tool.
- Ensures that the medication system and the resident health record are being used appropriately and adequately.
- Works with the consulting pharmacy to promote an optimal medication regime for each resident. Assists with scheduling pharmacy in-services to maintain education on medication management for medication aid staff.
- Plans, communicates, and coordinates with medical and/or other providers concerning the health care needs of the residents as needed and/or requested. Act as liaison between Community and outside health service providers.
- Monitor and Manage resident profile on EHR system to ensure compliance and accuracy of date entry.
- Plans and executes in-services and infection control as needed per regulations and as needed or requested by the use of a calendar.
- Supervises the limited nursing service of the Community, if provided.
- Communicates with the staff using the Staff Communication Binder.
- Communicate updates on care levels or change of conditions to the responsible party’s timely to ensure they are aware of residents’ well-being and needs.
- Performs all other duties assigned by any member of management.
Job Qualifications:
- Ability to utilize and apply basic management skills.
- Provides and coordinates resident care with an awareness of the principles of assisted living.
- Maintains adequate attendance, notifies supervisor if late or unable to work in accordance with personal policies, follows work schedule.
- Requires minimal supervision, is a self-starter and is flexible.
- Organizes and utilizes time appropriately, sets priorities and accomplishes assigned tasks.
- Displays mature behavior in attitude and speech and action and demonstrate consistently pleasant demeanor, tone of voice, supports the philosophy and approach used by the Community.
- Keeps resident information confidential, respects resident’s rights, respects privacy and the right to self-determination by the residents.
- Works cooperatively with the supervisors and co-workers.
- Abides by established policies and procedures of the facility. Looks for ways to improve facility function.
- Has the ability to stand for long periods of time on your feet and ambulate quickly.
- Is accurate, dependable, neat, and thorough in assigned tasks.
- Is effective in both verbal and oral communication.
Requirements:
- Tuberculosis (TB) certificate as required by the State.
- First Aid and CPR Certifications as required by the State.
- Criminal Background check cleared.
- Fingerprint clearance as required by the State.
- Must be at least 18 years of age, or minimum age as outlined in state regulations.
- Must be able to read, speak and write the English language.
- State Criminal Background Check and LIC 508 Criminal Record Statement
- LIC 503 Health Screening Report
Other Requirements:
- Previous experience working with the elderly population as an RN/LVN is preferred.
- Unencumbered RN/LVN license issued by the state in which employed is Preferred.
- Maintains CPR certification.
Universal Precautions:
Exposure to blood/bodily fluids is likely.
Physical Requirements:
Has ability to do occasional heavy lifting (up to 40 pounds).
