Job Description

Located in the heart of World Golf Village, Westminster St. Augustine is currently accepting applications for the position of full-time Director of Social Services in our Health Center. This individual will assure that the emotional and social needs of all residents are met and maintained on an individual basis in accordance with regulatory guidelines and our established standards and guidelines. This position will also perform clinical assessments for potential and current Health Center residents. At times, may also assist in liaise between community relations, discharge planners, and Physicians as well as support the Marketing efforts at the community.

ESSENTIAL JOB FUNCTIONS:

The Director of Social Services must comply with all applicable rules, policies, standards and guidelines related to employment within Westminster and its communities, including:

  1. Prepare comprehensive psychosocial assessments and care plans for all residents.
  2. During the Admission process tour facility and available accommodations with potential residents and family members. Review and obtain signatures for required documentation.
  3. Resident and family outreach – advocate for the residents and be the main contact for the family members.
  4. Access to community resources and present residents/family member with health care provider availability; includes home health care, hospice, equipment provider, etc.
  5. Manage discharge planning; responsible for appropriate discharges to a less restrictive setting (outside health services).
  6. Internal contact for resolving resident/family member grievances.
  7. As a Grievance Coordinator maintains a Grievance Log (accept/write grievances on behalf of party); conducts investigations and follow ups with resolutions. Conducts abuse prevention In-services for staff members.
  8. Completes QA and Audits. Monitors psychotropic medications, mood and behavioral symptoms. Obtain consent from residents or HC Surrogate for psych med use and maintain in psychoactive medications binder
  9. Works as community liaison for referrals from physician, hospitals, insurance based case manager, or marketing personnel on all potential sub-acute patients, ensures initial and on-going fiscal appropriateness of member admissions in conjunction with Business Office and Director of Nursing.
  10. Assists the community with tracking of authorization numbers of residents on Managed Medicaid and other Managed Care Insurance plans.
  11. Works with community to monitor and track resident's progress and communicate relevant information between resident, referrer, family, insurance company, and treatment team, communicates with the interdisciplinary member

    care team.

  12. Works with residents, families and other staff as needed to ensure that residents are evaluated for the appropriate level of care.

  13. Assists with coordination, monitoring, and communication of resident's progress and costs.

  14. Fosters positive relationships with insurance representatives and providers in the local market.

  15. Addresses quality measures issues for potential admissions and returning residents.

  16. Complete Pre-Admission process in EHR for new admissions.

ESSENTIAL QUALIFICATIONS:

Education & Experience: Bachelor’s Degree in Social Work or Sociology and 3 Years experience in skilled nursing facility or medically related area is required. Certification in Case Management or Geriatrics preferred. Must be CPR Certified. 

Physical and Environmental Requirements: Balancing, Kneeling, Reaching, Crouching, Pulling, Standing, Manual Dexterity, Pushing, Stooping, Grasping, Lifting Up to 40 lbs.

An individual in this position will be exposed to:
